The Summit Dental Plan is not insurance. The program is offered to individuals and families who do not have insurance, or who choose not to utilize their insurance. Summit Dental Plan benefits can only be used at Summit Dental P.C. ( Hackensack and Tenafly ) and are not transferable to another dental practice or dental specialty practice. Payment is always due at time of service.

Membership is for 1 year beginning on the enrollment date.
Membership dues are payable in full or in part upon enrollment and are non-refundable. Monthly payment plans require a credit card, the ability to auto bill the CC for 12 equal payments.
Cash, check or credit card payments are accepted for yearly subscriptions, payment plans can only be payed for by credit card.
Membership will automatically renew on the anniversary date unless a written request to cancel is received 30 days prior to the renewal.
Payments for additional dental services are the member’s responsibility. Payment is due on the date of service to qualify for the discounts detailed above.
Membership must be current to receive the discount.
It is the member’s responsibility to schedule and keep all appointments offered as part of the Summit Dental Plan.
Please notify our office at least 24 hours in advance if you must change a scheduled appointment if not a $50 cancellation fee will be incurred.
No deductibles, pre-authorizations, yearly maximums, waiting period or exclusions for elective care.
Plan exclusions include CT-scan imaging, Invisalign . SDP cannot be combined with any other promotions or discounts. This plan does not include treatment for gum disease (debridement, scaling, root planing). Once treatment is rendered, the fee is non-refundable or transferable. Plan and fees are subject to change.
